Fatty liver disease is one of the most common causes of insulin resistance and glucose intolerance, and also a major risk factor for advanced liver disease. Weight loss is a cornerstone of treating fatty liver disease. However, it has remained largely unclear whether the macronutrient composition of the weight loss diet affects the effectiveness of treatment. In this landmark study published in 2026, researchers compared three diets differing hugely in their macronutrient composition and foods consumed: a very-low-carbohydrate ketogenic diet, a Mediterranean diet, and a low-fat plant-forward diet. In this video, we’ll discuss this study and interpret it in the context of the existing literature, and share how it affects our recommendations for people with fatty liver disease and related metabolic issues, such as insulin resistance and prediabetes.
